Low-Cost Sports Physicals

Sports physicals are required every 2 years for students in grades 7-12 participating in school sports. Students should plan on getting a physical before the sports season begins to ensure that their paperwork is complete before the deadline date for the sport they wish to participate in.

There are many clinics in our area offering low-cost sports physicals. Please see the attached document for clinic information.

Students are required to have a signed copy of the School Sports Pre-Participation Examination Form on file in the school office BEFORE they can begin practice for a sport. The physical form is attached. Students who have a valid physical on file in the school office do not have to get another one to participate. Sixth graders do not need a physical.
